Ann D., a 64-year-old U.S. Army veteran and devoted caregiver to her mother, living with dementia, was admitted to The Monarch at Brooklyn Rehabilitation and Nursing Center on April 30, 2025. She had been hospitalized at SUNY Downstate Medical Center due to paroxysmal supraventricular tachycardia (SVT) and a sudden decline in mobility, requiring skilled inpatient rehabilitation before safely returning home. Ann lives with and provides full-time care for her mother, so regaining strength and independence was essential for both of their well-being. 

When Ann first arrived, she expressed concerns about her food, environment, and overall comfort. These details truly mattered to her, and they were heard. Within the first 48 hours, the concierge team, Assistant Director of Nursing, and Food Service Director collaborated to make her feel at home. A custom menu was created to meet her personal preferences and dietary needs, setting the tone for the individualized care she would receive throughout her stay. Ann was warmly welcomed by clinical and therapy staff who immediately began working with her to evaluate her needs and build a care plan focused on regaining her strength and mobility. Her goals were clear: walk independently, climb stairs, and return home to her mother with confidence. 

Over the following weeks, Ann made tremendous clinical progress. With the help of our skilled therapy team, she advanced from being unable to walk to ambulating over 200 feet with a rolling walker and completing eight stairs independently—a milestone that ensured she could safely return to her second-floor apartment. Along the way, Ann developed strong connections with the team members who supported her, from therapy to dietary to concierge. Each department played a role in creating a calm, supportive environment where she felt respected, seen, and empowered. Ann shared with us, “Everyone here truly listens. From the food I eat to how I feel each day, I never felt like just another patient.” 

Ann is scheduled for discharge on July 9th. She will return home with restored strength, renewed confidence, and a deep appreciation for the care she received at The Monarch. Continued clinical support has been arranged to ensure a smooth transition back into her role as both veteran and caregiver.
